2008 Trust-sponsored open enrollment is May 5-16, 2008. New coverage will begin July 1, 2008 (supplemental life will begin on July 1 or August 1).
Benefit
Changes
To review the changes upcoming July 1, check the Update section. Information regarding Trust benefits can be
found
in benefit plans, with special pages
dedicated to the legal plan and the portability
feature of the supplemental life plan. The Trust provides dental, vision, basic life insurance and Working
Solutions at no cost to you.
Look to our Links
for a quick connection to those insurance companies with websites.
Hyatt, VSP, MetLife and EyeMed are all there. Preferred Choice (MetLife),
EyeMed and VSP are all network-based plans, and you may want
to review the networks when trying to decide whether to keep
or change your current plan.
You can review legal plan information and even enroll on the Hyatt website.
Get your Enrollment Highlights
Booklet here. It has all the forms needed for enrollment additions, changes or drops, except the supplemental life insurance form. All members may take advantage of Working Solutions, supplemental life insurance and the legal service plan.
Only members with a year or more of State service are eligible for dental, vision or basic life (plus AD&D) insurance.
You'll
probably want to review the plan booklets in conjunction with your Enrollment
Highlights Booklet: The plan booklets have much more detail than that in the Highlights Booklet. The
Plan Booklets are being updated again, with the most current revisions to be posted soon. The booklets are as follows:
- One Year - Dental, vision and basic life insurance (accidental death and dismemberment insurance too)
- New Hire - Working Solutions, supplemental life insurance and the legal service plan.
