Vision

UBT offers you and your family high-quality vision plans intended to meet your needs. When you enroll, you can choose from either of UBT’s visions plans (if you are eligible). Unlike the medical plans, there are no restrictions on your plan choices and you pay no premiums.

Choose from either of the UBT vision plans, both allow you the freedom to visit the licensed provider of your choice.  But remember network availability varies by location and is an important feature of the plans.  If you visit a provider that is not in the network, you will receive a lower level of benefits than with a network provider.  See the plan booklet for additional information.

Options

  • EyeMed Vision Care Plan (EyeMed) (Select Network) EyeMed provides high-quality eye care and materials to members through well-known locations that include: LensCrafters, Sears, Target Optical and participating Pearle Vision Centers. The EyeMed network also offers many independent private practice providers. The network is spread throughout Ohio. To obtain a provider list for EyeMed, visit the EyeMed website or call 866-723-0514.
  • Vision Service Plan (VSP) (Choice Network) The Vision Service Plan (VSP) has a national network of doctors who provide high-quality eye care and materials to members. The VSP network is spread throughout Ohio and is available in all 88 counties. To obtain a provider list for VSP, visit the VSP website or call 800-877-7195.

Benefits are available every twelve (12) months, for you and your eligible dependents.  However, exam and lens benefits may be available sooner in the rare case of a sizable prescription change. Please see the section on Early Coverage in the plan booklet. Check your vision provider’s website frequently for discounts and special offers.

Discount Plans

After you have used the benefits outlined above, discounts for professional services and materials by network providers may be available. Both EyeMed and VSP offer 20% off the frame cost over the plan’s frame allowance, which may be used in conjunction with your Trust benefits. For more information, see Additional Discounts in the plan booklet. Check your vision provider’s website frequently for discounts and special offers.

Filing a claim

You are responsible for filing your claim if you receive vision care from a provider who does not participate in your plan’s network.

EyeMed

For vision care from a non-network provider, you must call EyeMed first for a claim form. You must pay the provider in full, then, submit itemized receipts in writing within six months of service completion.

Vision Reference Cards

A card is not required for service at your vision provider’s office. Simply give your State of Ohio ID User number at the time of service, and your provider will do the rest.

UBT heard your request for a simple, easy to use reference card to keep your benefit information on hand. You now have an option to print reference cards for you and your family’s vision visits.

For VSP

Visit VSP’s website register and create a password, and print a card with your name and group number.
